Youville Place performs an assessment on all prospective residents before their acceptance for residency, utilizing a medical information form provided by the prospective resident’s healthcare provider. The medical information form must be completed within three months prior to assessment. The prospective resident also meets with a Youville Place nurse for an initial assessment and review of personal data.
The prospective resident must meet the following criteria to be considered for residency:
- Financial
Have the financial resources to pay the assessed fees.
- Cognitive
Be alert and oriented to person, place and time.
- Functional
Able to transfer with moderate assistance.
Able to manage personal care with or without assistance.
Able to manage incontinence with staff assistance and/or use of incontinence control products.
- Pyschosocial
Can manage behavior appropriately in a community setting.
